This survey is sponsored by Northeast Delta Dental.  The purpose of the survey is to understand the knowledge, attitudes, and practices of dentists and physicians as related to the integration of oral health and medicine.  The survey explores the role of dentists and physicians through questions about training, appropriateness of care, and current practices.
